Couples and Marital Counselling
Couples and marital counselling is a structured form of talk therapy in which both partners meet with a trained counsellor to improve communication, rebuild trust and work through the problems that are straining their relationship. At Calida Rehab, couples counselling is offered at our Pune centre and our Mumbai Region – Karjat centre. It supports partners whose relationship has been affected by addiction, anxiety, depression or long-standing conflict and mistrust.
- Best for: couples facing conflict, broken trust, poor communication, or strain caused by addiction or mental health problems
- Approach: joint sessions with both partners, with individual sessions where needed, alongside medical care when the treating doctor finds it appropriate
- Methods used: communication skills training, behavioural couples therapy, CBT-based problem solving, relapse prevention with the partner, psychoeducation
- Format: assessment, safety screening, goal setting, regular joint sessions, progress review, aftercare planning
- Not a first step when: there is violence, abuse or coercion at home (safety planning comes first)
- Where: Pune centre and Mumbai Region – Karjat centre
- Last updated: November 2026
- Couples counselling is a neutral, guided therapy for two partners. The counsellor works on the patterns between partners and does not decide who is to blame.
- Addiction and relationship distress feed each other. Treating the couple, and not only the person with the addiction, is a recognised part of care.
- A 2008 meta-analysis of randomised trials found that behavioural couples therapy improved relationship satisfaction and, at follow-up, reduced substance use and its consequences compared with control treatments.
- Most research programmes run about 12 to 20 weekly sessions over three to six months, though each couple’s plan is individual.
- If there is violence or abuse at home, safety and individual support come before any joint sessions.
What Is Couples and Marital Counselling?
Couples counselling, also called marital counselling or marriage counselling, is a form of talk therapy in which two partners work with one trained professional in a private, confidential setting. The counsellor does not take sides. The aim is to help both partners understand each other, speak openly and solve problems together rather than argue around them.
Many couples come when arguments keep repeating, when trust has been broken, or when one partner’s drinking, drug use or mental health condition has changed life at home. Others come before a crisis, simply because they want to communicate better. Counselling can help in both situations.
Sessions are tailored to each couple. The plan is set after a full assessment and is based on the history of the relationship, each partner’s health, safety needs and the goals the couple agree on.
Why Couples Counselling Is Important
Addiction and mental illness rarely affect only one person. Partners often carry fear, anger, exhaustion and financial stress, while the person in treatment may feel guilt and shame. Without a safe place to talk, these feelings turn into silence or conflict, and both can raise the risk of relapse or a return of symptoms.
Couples counselling gives both partners a guided space to be heard and to learn new ways of responding to each other. At Calida Rehab it works together with individual counselling, group work and family therapy, so that the whole circle of support is involved in recovery.
Couples Counselling vs Marital Counselling vs Family Therapy vs Individual Counselling
Short answer: couples counselling and marital counselling are essentially the same approach. Marital counselling is the term often used for married partners, while couples counselling covers any committed pair. Family therapy involves wider family members, and individual counselling is one-on-one.
| Type | Who attends | Main focus | Best when |
|---|---|---|---|
| Couples / marital counselling | Both partners | Communication, trust, conflict and shared recovery plans | The relationship itself is under strain or is affected by illness |
| Family therapy | Patient with parents, siblings, spouse or children | Family roles, boundaries and support for recovery | Several family members are involved in the problem or the solution |
| Individual counselling | One person | Personal triggers, emotions, coping skills and goals | The person needs a private space or is not ready for joint sessions |
| Group therapy | Peers with a clinician | Shared experience, accountability and peer support | Isolation or shame is a major barrier |
These options are not either-or. Many couples at Calida Rehab combine joint sessions with individual counselling so that personal issues and relationship issues are both addressed.
Signs Your Relationship May Need Couples Counselling
Short answer: consider couples counselling when the same arguments keep returning, when trust is broken, when one partner’s addiction or mental health condition dominates home life, or when you feel more like strangers than partners.
- Arguments repeat without resolution, or end in shouting, silence or leaving the house
- One or both partners avoid talking about important topics to prevent a fight
- Trust has been damaged by lying, secrecy, hidden drinking or drug use, or financial concealment
- One partner has become the carer, monitor or rescuer of the other
- Emotional or physical distance has grown, including loss of closeness and affection
- Money, parenting, in-laws or household duties cause constant conflict
- One partner is returning home after rehab or a hospital stay, and roles need to be rebuilt
- A partner feels anxious, hopeless, constantly on guard or unable to sleep because of the situation at home
- Either partner has started to think about leaving, or the relationship feels like it is ending
These are signs, not a diagnosis. A short assessment with our team can help decide whether joint sessions, individual counselling or a combination is the right starting point. If any of these signs come with fear for your safety, read the section below on when couples counselling is not the right first step.
How Couples Counselling Works
Couples counselling follows a clear structure, and the pace is set with the couple. The first sessions focus on understanding the problem and building safety and trust in the room. Skills and change come step by step after that.
Typical stages of care
- Assessment: the counsellor meets the partners together and, where helpful, each one separately. The history of the relationship, any substance use or mental health concerns and, importantly, safety at home are reviewed.
- Goal setting: both partners agree on clear, realistic goals, such as calmer discussions, rebuilding trust or planning life after treatment.
- Joint sessions: partners practise listening, expressing needs without blame and solving problems as a team.
- Individual support: where needed, each partner also receives one-on-one support, so that personal issues are not left unaddressed.
- Review and aftercare: progress is tracked with the wider care team, relapse prevention is planned together, and follow-up sessions are arranged.
Change takes time and differs for every couple. Many partners notice calmer conversations within the first few weeks, while rebuilding trust usually takes several months of regular sessions and follow-up.
Concerns We Address with Couples Counselling
Couples counselling at Calida Rehab is available at our Mumbai-region centre in Karjat and our Pune centre for partners dealing with:
Addiction and Its Effect on the Relationship
Alcohol or drug use can bring lies, broken promises, money problems and fear into a relationship. Counselling helps partners talk about what happened, set healthy boundaries and support recovery without blame. It is combined with medical care and relapse prevention. See our pages on alcohol addiction treatment and drug addiction treatment.
Anxiety, Depression and Stress in a Partner
When one partner is living with constant worry or low mood, the other may feel shut out or helpless. Counselling helps both understand the condition, reduce misunderstandings and respond with support. It is combined with medical review for anxiety disorders and depression when needed.
Communication Problems and Constant Conflict
Repeated arguments, silent treatment, criticism and defensiveness can wear a relationship down. Partners learn calmer ways to raise issues, listen without interrupting and repair after a disagreement.
Broken Trust and Emotional Distance
Secrecy, lying or long periods of withdrawal can leave partners feeling like strangers. Counselling gives a safe, guided setting to talk about hurt, rebuild trust step by step and reconnect. Our interpersonal therapy page explains how relationship patterns and mood are linked.
Benefits of Couples Counselling
Couples counselling gives partners a steady, structured place to work on the relationship and on recovery together. Many couples notice gains as sessions continue.
Key advantages
- A neutral, guided space where both partners are heard
- Clearer, calmer communication
- Better understanding of how addiction or mental illness affects each partner
- Rebuilding of trust at a realistic pace
- Healthier ways to handle conflict and stress
- Stronger partner support for recovery and lower risk of relapse
- Less guilt, blame and resentment
- Shared plans for money, routines and family life after treatment
Results vary from couple to couple, and they are strongest when both partners attend regularly and are supported by the clinical team.
Does Couples Counselling Work? What the Research Shows
Short answer: yes, for many couples. The strongest evidence is for behavioural couples therapy when one partner has an alcohol or drug use disorder. Outcomes vary, and attendance and safety matter.
A meta-analysis published in 2008 in Clinical Psychology Review pooled randomised controlled trials of behavioural couples therapy for alcohol and drug use disorders. It found that the therapy improved relationship satisfaction soon after treatment. At follow-up it also reduced how often the person used substances and the harm caused by use, compared with control treatments, most of which were individual CBT-based.
Authors of a widely cited clinical review described behavioural couples therapy as the couples and family approach with the strongest empirical support in substance use treatment. Some trials have also reported lower intimate partner violence and better emotional and behavioural health in children of treated couples.
What the Evidence Does Not Say
- It does not promise that every relationship will be saved. Counselling helps couples make informed choices, including, in some cases, a safe and respectful separation.
- Most trials have included couples where only one partner has a substance use disorder, and many were run by the teams that developed the therapy, so reviewers describe the evidence as moderate to strong rather than final.
- Evidence for other approaches, such as emotionally focused therapy, is mostly for relationship distress in general and not specifically for addiction.
- Results depend on regular attendance, honesty and, above all, on safety at home.
Because of this, our team treats couples counselling as one part of a full plan that includes medical care, individual counselling, group therapy and aftercare.
Common Couples Counselling Approaches
The approach depends on the couple’s concerns, history and readiness. The counsellor chooses one method or a combination of methods. Here are the most common ones:
Behavioural Couples Therapy
Where one partner is recovering from addiction, this approach involves the other partner in the recovery plan. Couples agree on supportive routines, practise positive interaction and learn how to respond to cravings, slips and early warning signs together.
Communication and Problem-Solving Skills
Partners practise speaking about needs without blame, listening fully and solving problems step by step. These skills build on the principles of cognitive behavioural therapy (CBT), which links thoughts, feelings and behaviour.
Relapse Prevention with the Partner
Couples learn to spot high-risk situations and make a shared plan for them. Read more on our page on relapse prevention therapy.
Emotion Regulation Skills
Skills for managing strong emotions, tolerating distress and staying calm during conflict are taught where the treating team finds them suitable. Learn more about DBT.
Individual and Family Sessions Alongside
Personal issues are addressed in individual counselling, and wider family relationships are supported through family therapy and group therapy when needed.
Counselling should be done by a trained professional. Always ask about training before you start, and never begin or stop any medicine without your doctor’s advice.
When Couples Counselling Is Not the Right First Step
Short answer: couples counselling is not the right first step when one partner is unsafe, when a partner is intoxicated or in withdrawal, or when someone is in acute mental health crisis. In these situations the first step is safety and medical or individual care.
- Violence, abuse or coercive control: joint sessions can put the less powerful partner at risk, since speaking openly may lead to retaliation. The counsellor first focuses on safety planning and individual support. Help is available through the Women Helpline on 181 and emergency services on 112.
- Active intoxication or withdrawal: a person who is intoxicated or in withdrawal cannot take part in therapy safely. Medical care such as detoxification comes first.
- Acute mental health crisis: severe suicidal thoughts, psychosis or severe mania need urgent psychiatric care before relationship work.
- One partner is being pressured to attend: change cannot be forced. Individual counselling can be offered to the willing partner meanwhile.
Moving to joint sessions later is usually possible once the immediate risk has been addressed.
Myths vs Facts About Couples Counselling
Many couples delay counselling because of fear, pride or misunderstanding. Knowing the facts helps in making an informed decision.
| Myth | Fact |
|---|---|
| Couples counselling is only for couples about to separate. | Many couples come to communicate better, rebuild trust or prepare for life after treatment. Early help often works best. |
| The counsellor will take sides and decide who is to blame. | A trained counsellor stays neutral and works on patterns between partners rather than blaming one person. |
| If only one partner has the addiction or illness, only that partner needs help. | The partner is affected too, and involving them supports recovery. Both partners can benefit from support. |
| Counselling will fix the relationship in a few sessions. | Change is gradual. Regular sessions and practice at home are needed, and both partners must take part. |
| Couples counselling is right in every situation. | Where there is violence or abuse, safety comes first. Individual support and safety planning are needed before any joint sessions. |
Key Features of a Good Couples Counselling Program
Many centres offer counselling, but good couples care needs trained professionals and a safe setting. A good couples counselling programme has these features:
- Led by qualified psychiatrists, psychologists and trained counsellors
- Full assessment of both partners and a clear treatment plan
- A neutral counsellor who does not take sides
- Safety screening before joint sessions begin
- Clear confidentiality rules explained to both partners at the start
- Evidence-based methods such as behavioural couples therapy and CBT-based skills
- Joined-up care for co-occurring addiction, anxiety or depression
- Doctor-supervised medication review when needed
- Family involvement, education and aftercare planning
- Individual counselling, group therapy and healthy routines alongside couples sessions
Relationships touch every part of life, so combining couples sessions with other therapies helps change last. Not sure who to see first? Read our guide on psychiatrist vs psychologist vs counsellor.
How to Prepare for Your First Couples Counselling Session
Short answer: decide on a few goals, gather relevant medical information, agree on basic ground rules with your partner, and come ready to listen as well as speak.
- Talk to your partner first. Invite them gently and explain that the aim is to improve things together, not to blame.
- Write down your main concerns. Choose two or three topics that matter most, and what a better situation would look like.
- Bring health information. Note any current medicines, past diagnoses, treatment records or hospital reports.
- Be ready for honesty. Sessions work best when both partners can describe facts and feelings without exaggeration.
- Agree on ground rules. For example, no interrupting, no raised voices, and a break if either person feels overwhelmed.
- Ask practical questions. Ask about session length, number of sessions, fees, confidentiality and how progress is reviewed.
It is normal to feel nervous or even a little worse after the first session, because difficult topics are being named for the first time. Tell the counsellor if the pace feels too fast.

How to Choose the Right Couples Counselling Centre
Choosing the right facility plays an important role in recovery and in the health of the relationship. Here are the steps to follow:
- Make a list of reputed centres offering couples or marital counselling in nearby locations.
- Check that the counsellors and psychologists are licensed and trained to work with couples.
- Ask whether the counsellor stays neutral and how both partners are treated equally.
- Check that the centre can treat co-occurring addiction, anxiety or depression at the same time.
- Ask how safety is screened and how privacy and confidentiality are protected.
- Ask about individual sessions, family involvement, relapse prevention and aftercare support.
- Finally, look for real reviews and speak to families who have used the service.
Understanding the therapies used in rehab can also help families understand treatment decisions and expectations.
More Questions About Couples Counselling
How much does couples counselling cost in Pune or Mumbai?
Fees depend on the format (outpatient sessions or part of a residential programme), the number of sessions and the clinician. Please call the Pune or Karjat branch for current fees and what they include.
How many sessions of couples counselling are needed?
It varies. Research programmes of behavioural couples therapy usually run about 12 to 20 weekly sessions over three to six months. Some couples need fewer, and others need more. Your plan is reviewed regularly with the treating team.
Can couples counselling be done if only one partner is in rehab?
Yes. Partner involvement is planned around the patient’s stage of treatment, and the team advises when joint sessions are appropriate. The partner outside can also receive education and individual support.
What if my partner refuses to come?
You can begin with individual counselling to understand the situation, learn healthy boundaries and look after your own wellbeing. Avoid ultimatums. A calm, non-blaming invitation to try one session often works better.
Can couples counselling save a marriage?
It cannot guarantee any result. It helps partners understand the problems, communicate and make informed choices, whether that means rebuilding the relationship or, where needed, separating in a safe and respectful way.
How is couples counselling different when addiction is involved?
Addiction-focused couples work adds recovery goals to relationship goals. It covers rebuilding trust, handling cravings and triggers together, planning for slips, setting boundaries without blame, and supporting the partner’s own health, along with medical care for the patient.
Is it normal to feel worse before feeling better?
Yes. Naming painful topics for the first time can cause temporary discomfort. A trained counsellor paces the work and adjusts it. Tell your counsellor if you feel overwhelmed.
Can couples counselling help after infidelity?
It can offer a structured, safer space to talk about what happened, the hurt it caused and whether and how to rebuild trust. Outcomes vary, and the counsellor will first make sure that both partners feel safe to take part.

Couples Counselling at Calida Rehab - FAQs
What is couples counselling?
Couples counselling is a structured form of talk therapy in which partners meet with a trained counsellor to improve communication, rebuild trust and resolve conflict.
Is marital counselling the same as couples counselling?
Largely yes. Marital counselling usually refers to married partners, while couples counselling is open to any committed partners. At Calida Rehab the approach is the same and is adapted to each couple.
Can couples counselling help when addiction has damaged the relationship?
Yes. Involving the partner is a recognised part of addiction care. It can improve communication, rebuild trust and support relapse prevention, and it is combined with individual counselling and medical care.
Do both partners have to attend?
Joint sessions work best when both partners take part. If one partner is not ready, the other can begin with individual counselling, and the counsellor can guide how to invite the partner later.
How long does couples counselling take?
It varies by couple. Many partners notice calmer conversations within a few weeks, while rebuilding trust usually takes several months of regular sessions and follow-up.
Is couples counselling suitable if there is violence or abuse at home?
Not as a first step. Safety comes first. Where there is violence or abuse, the counsellor focuses on safety and individual support before any joint sessions. If you are in danger, call 112 or the Women Helpline on 181.
